La stratégie d’approche auprès des femmes n’est pas encore bien définie, de même auprès de l’ensemble de la population.  Appui auprès de l’association TUNFA : Réunions, compte rendus de réunions, suivi du programme, propositions d’activités. il faut constamment les dynamiser. 4- Formation matrone et infirmiers/ SF de brousse : Ce volet est mis en place par la SF actuelle. Il s’agira de planifier ensemble les activités. Le CM/ Médical devra contacter les autorités sanitaires locales pour le calendrier des activités en brousse. Relations internes : Il devra superviser l’ensemble de l’équipe AMI, y compris les deux responsables locaux. Il devra maintenir une collaboration avec la présidente de TUNFA, qui ne s’implique pas du tout dans les activités.     Mise en place des programmes de formation : ● se documenter sur les programmes nationaux, et formations d’autres ONGs validées par le Ministère de la Santé. Ce qui veut dire accepter d’aller sur Niamey chercher l’information quand cela s’avère nécessaire. ● élaboration des programmes de formation en collaboration avec le médecin responsable du district et les responsables de la DRSP. ● faire valider les programmes de formation par les autorités locales avant de commencer la formation. L’idéal serait de le faire aussi auprès du Ministère de la Santé mais cela prend du temps. ● connaître et suivre le budget des formations. Sans faire la comptabilité de ces programmes, il est important que la sage femme connaisse les grilles budgétaires et bailleurs qui financent ces formations et suive les dépenses ; ceci facilitera et allègera le travail de l’administratrice, et ne pourra que renforcer la complémentarité de l’équipe. De plus, il me semble dommage que la sage femme ne s’intéresse qu’au coté médical de son travail ; à mon sens, elle se prive de toute une partie des formations, et dans le cadre d’une coordination, elle peut – être amenée à aborder ce volet. ● élaborer des outils pédagogiques ● élaborer des outils de suivi de ces formations ● rendre des rapports de ces formations

    En conclusion, il me semble important que la sage femme assume l’ensemble des aspects de l’élaboration de ses programmes de formation et de suivi. Travail d’équipe : - Nous ne sommes que trois, et chacune avec de grosses tâches de travail. Il est important que l’information circule, mais ce n’est pas toujours facile quand la sage femme part en brousse pour les formations, le CM sur Niamey…d’où l’importance de faire régulièrement rapports et autres et de les soumettre à l’ensemble de l’équipe. - Quand le CM est indisponible pour assister à une réunion, c’est à la sage femme d’y assister. Si elle-même est indisponible c’est à l’administratrice d’y assister. Il est important de montrer à l’extérieur cette complémentarité, nous en serons nous-même plus soudées et plus crédibles pour nos partenaires. - Ce travail d’équipe intègre les activités de TUNFA, qu’il nous faut vigiler, et qui requière chacune de nous trois. - Collaborer avec les partenaires, que ce soit autorités locales, associations et ONGs. Nous avons de bons contacts avec l’ensemble des ONGs, et il nous faut collaborer de plus en plus dans le sens d’une recherche permanente de complémentarité avec ces partenaires, si nous voulons être crédibles auprès des bailleurs potentiels. - Représenter AMI quelque soit le lieu où nous sommes, le type de mission, même si la mission est dirigée par une association ou autre ONG.

    Activités par ordre chronologique que la sage femme aura à mettre en place:

    ● Partenariat AMI/ TUNFA : 1- Formation des infirmiers de 5 CSI : Cette formation est prévue en 2 cessions de 5 étudiants chacune, de façon à former l’infirmier et le major de chaque CSI. La première cession est prévue du 25 au 30 juin. La deuxième cession est prévue du 2 au 7 juillet. Il est fort possible que la première cession soit repoussée à cause des JNV, dont la date vient d’être nouvellement fixée. - but de la formation : renforcement des compétences en urgence obstétricale

    2- Formation de matrones de brousse : Christiane a déjà formé 20 femmes et est actuellement sur la formation de 10 autres femmes. Les sites où a eu lieu des formations : Egendawael, Elmécki, Ingall (actuellement) Aderbissinat prévue fin mai- début juin.

    Formation de matrones de brousse de Tchirozérine : Cette formation est prévue pour début juillet, mais il est fort possible qu’elle soit avancée à cause du report de la formation des infirmiers. - 10 matrones à recenser (10 jours en brousse pour recensement, sensibilisation, identification). - Formation théorique au sein du CSI : 6 jours - Formation pratique à la maternité du C.H.D : 6 jours.

    Suivi et supervision des 50 matrones formées : Cette supervision doit se mettre en place avec l’équipe de TUNFA et les autorités sanitaires du district. Nous pensons qu’il est préférable dans un premier temps d’assurer cette supervision sur le lieu de vie de chaque matrone, ceci pendant 4 mois. Puis réfléchir sur une supervision possible au niveau de chaque CSI. Ce qui voudrait dire que l’on donnerait rendez-vous aux matrones tous les mois au niveau de leur structure de référence. Cela sera certainement difficile à mettre en place. Il faudra donc le discuter avec le responsable du district qui est de bons conseils.

    Novembre : Deuxième cession de formation de 50 matrones. Ces formations sont prévues sur les 5 mêmes CSI, mais il serait pertinent de voir avec le district d’autres zones prioritaires et envisager de former ailleurs, car nous avons déjà des demandes sur d’autres CSI. Donc évaluer d’autres sites avant de lancer cette nouvelle cession de formation.

    3- Supervision des activités de TUNFA sur Agadez :

    IEC/ SMI : Une infirmière de TUNFA, salariée par AMI, est chargée de ce programme. Elle a élaboré un guide de sensibilisation. Tous les mois elle anime 15 quartiers sur un même thème, qui change chaque mois. A partir de mi mai, elle va assurer une permanence, 2 après midi par semaine, dans un local réhabilité par AMI.

    IST/ SIDA : Le programme n’a pas encore commencé. Un infirmier a été recruté et est détaché par AMI auprès de TUNFA. Il vient de suivre une formation auprès du PNLS. Il doit mettre sur pied une enquête auprès de femmes sur Agadez, puisque ses premières activités sont prévues auprès des femmes, par le biais de causeries de quartiers. Programme à élaborer en intégrant les hommes et les religieux pour mieux atteindre les femmes. Par la suite il faudra élaborer un programme de sensibilisation auprès des scolaires et lycéens. Permanence de deux après midi au niveau du local

    Le travail de la sage femme doit être de conseil, surtout auprès du responsable sida qui a un programme difficile à élaborer et mettre en place. Donc tous les outils pédagogiques et IEC sont les biens venus. TUNFA a à présent une télé et une série de K7 vidéo sur la SMI et le sida. Un rétroprojecteur peut être emprunté à la DRSP.

    ● AMI

    Maternité du C.H.D : Entre deux supervisions, maintenir le contact avec la maternité, car là notre présence n’a pas était très positive, et le personnel n’est pas très coopérant. Envisager de revoir un module de formation (à discuter), à élaborer avec la sage femme surveillante de la maternité, sage femme responsable au sein de la DRSP, en conformité avec la politique de santé (formation de formateurs …) et revoir une possible formation continue. De toute façon, notre présence est « continue » sur 3 ans, entre deux formations en brousse.
